Yearly Angels, Inc. is a 501(c)(3) public charity organization formed to provide a financial gift to individuals or families who are either (a) living with Multiple Sclerosis or living in a household with someone who is living with Multiple Sclerosis or (b) have been impacted by economic hardship and have a gross income at or below 175% of the Federal Poverty Level and have a need of extra assistance during the Christmas holiday season to meet various needs, including paying their bills, medicine, getting to and from their medical appointments, or purchasing basic necessities.

Yearly Angels, Inc. has created two programs to help individuals in need. They are called the Multiple Sclerosis Help Program(MSHP) and the Income Help Program(IHP). When you fill out the application, check the box for which program you apply for the gift award.

Multiple Sclerosis Help Program (MSHP) is designed to help individuals or families who have been diagnosed with Multiple Sclerosis and need extra assistance during the Christmas holiday season to meet various needs, including paying their medical bills, buying medicine, paying household bills, buying toys for their children, clothes, food, or purchasing basic household necessities by providing a gift from One Dollar ($1) to One Thousand Dollar ($1,000.00) to each individual/family recipient.

Income Help Program (IHP) is designed to help individuals or families who have been impacted by economic hardship and need assistance during the Christmas holiday season to meet various needs, including paying their bills, buying toys for their children, clothes, food, or purchasing basic household necessities by providing a stipend award from One Dollar ($1) to One Thousand Dollars ($1,000.00) to each of the individual/family recipients. To be eligible, a household must have a gross income at or below 175% of the Federal Poverty Level and responsible for the bills.

The Maximum Income Limits

The following are the 6 documents you must submit with the application by email. Please read the list carefully. You will need to supply all documents upfront to be considered. There is NO guarantee you will be chosen even if you supply the required documents, but if you don’t, you will not be considered. Due to a lot of fraud, this is a way to minimize scams and ensure you qualify. If you are chosen as one of the applicants for receiving funds from $1 up to $1,000, you will be notified by December 12. If you do not hear from Yearly Angels by December 12th, then you were not chosen. You will send copies, not the originals of 1 through 5. Still, for number 6, the Affidavit of Eligibility and Liability Form-Yearly Angels needs the original document to yearlyangels@gmail.com.

1. Proof of Identification (i.e., Driver’s License)
2. Proof of Income (Recent Pay stubs, income verification letter, etc., reflecting all earned income information for everyone 21 years and older who resides in the household).
3. Current bill (medicine, rent, car note, or basic necessity) that would be paid with the stipend. Yearly Angels reserves the right to pay a portion or all of the stipend to a third party directly for recipient’s medical and/or basic necessity bills.
4. Proof of U.S. Citizenship of Legal Residency Status (i.e., U.S. Passport or Birth Certificate).
5. Proof you have Multiple Sclerosis. (Only apply to MS patients).
6. Affidavit of Eligibility and Liability Form. It has to be signed.
